Membership Rates
Primary Care for Individuals:
All new members will pay a one-time ENROLLMENT FEE of $100 at the first visit. You will NOT be billed today.
Children 0-18 -$30/month with at least one parent enrollment *At this time we are not able to provide routine vaccinations, call to discuss how we can help arrange these for you.
Adults 20-50 - $60/month
Adults 51-64 - $85/month
Adults 65+ - $110/month
Regular billing will start the month you enroll regardless of the first visit date:
Primary Care for Business:
All new members will pay a one-time ENROLLMENT FEE of $100 at the first visit. You will NOT be billed today.
Business owners can enroll at least 5 of their employees at only $60 per month per adult (regardless of age) and $30 per month per child (with a paying adult). Under 5 employees, monthly fee is based on age but enrollment fee is waived.
